Before
submitting any forms to IRB-01 you should be intimately
familiar with our requirements. Please read our
instructions for completing forms as well as our
"Submitting Paperwork
to IRB-01" page before submitting any paperwork.
Some forms (e.g. the Informed Consent form) have
their own specific instructions. You are required
to be familiar with any pertinent information prior
to submitting these forms.

FORMS MUST BE TYPED!
Failure
to complete forms properly may result in your submission
being returned to you!
NOTE:
some types of submissions (for example, reporting
a protocol deviation) do not have a required form
available. If you need to submit information to
the Board and a form is not available, please provide
a cover letter detailing the content of your submission.

Fixing
Microsoft Word to correctly print page numbers on
the Informed Consent Form or other IRB documents.
Older
versions of Microsoft Word (e.g. Word 97) sometimes
have the tendancy to show page numbers correctly
on the screen, but do not print the numbers correctly.
For example, in our Informed Consent Form the footer
is set up to display page numbers as :

This appears correctly on screen, but when printed
does not properly list the total number of pages
on each page. In the example below, it should be
5 total pages.

This
problem can potentially be resolved for free by
visiting the Microsoft.com Office
Download Center.
In
particular, there are three updates to Microsoft
Office that sometimes correct this problem:
- Office
97 Service Release 1 (SR-1)
- Office
97 Service Release 2 ValuPack
- Office
97 Service Release 2b (SR-2b)
If
you download these Service Releases and they do
not correct the problem, you only have two options:
1.
Print your form on a lab computer (such as in the
Medical Sciences Library in the Communicore Building)
or
2.
Upgrade your Microsoft Office package to Office
XP/2003. The University of Florida standard license IS FREE for UF faculty, staff, and students. Contact your departmental IT professional for more information.
Information
for updating software at the VA is not currently
available.
